Construction site management is not measured solely by the ability to advance the work, but primarily by the ability to anticipate situations that may develop into failures. A significant portion of serious incidents does not stem from a single exceptional mistake, but rather from the accumulation of known gaps that were not addressed in time.
The following document presents common failure scenarios that should be anticipated in advance, with the aim of sharpening critical control points and preventing operational, legal, and criminal exposure.
1. Commencement without written approval
Starting work without written approval from the client is not a technical defect, but a fundamental failure in the prerequisite conditions for initiating the project.
Written approval is not merely a formal document; it reflects that the client has reviewed the initial conditions and concluded that work can begin in compliance with regulatory requirements. When work begins without such approval, it means the project has been launched without a clear control point and without confirmation that the prerequisite conditions have indeed been met.
In the event of an incident, the question will not only be whether the conditions were actually met, but also how the decision to commence work was made, who made it, and on what basis. In the absence of documented approval, it becomes very difficult to demonstrate that the project was initiated within an orderly management framework.

2. Safety controller quits role in the middle of the project
When a safety inspector ceases to serve in their role and no replacement is appointed immediately, a significant gap is created in the project’s control mechanism.
The regulations stipulate that in such a situation, the duties assigned to the inspector transfer to the client. This is not merely a formal implication: the developer may find themselves required to assume operational responsibility in practice, without having an established mechanism for execution, oversight, and documentation.
In the event of an incident, the key question will be how the site continued to operate without an active control function, and what measures were taken during that period to identify risks and supervise the implementation of the safety plan.

On a construction site whose area according to the building permit is at least 15,000 square meters, appointing a site manager is not a recommendation but a regulatory obligation.
The absence of a site manager is not a minor anomaly, but a structural failure in the control system of a complex project. This means that the project is being run without an additional layer of control designed to deal with complexity, multiple contractors, concurrent work, and changing risks.
In the event of an incident, not only the question of appointment will be examined, but also the gap between the complexity of the project and the actual control mechanism.
3. A large website runs without a webmaster

The foreman is the central factor in the actual management of safety on the site. When he ceases to serve in his position, the construction operator is not permitted to continue work until a new foreman is appointed, except in exceptional cases stipulated by law.
In practice, continuing to work without a foreman is not only a regulatory violation, but a situation where there is no entity holding day-to-day operational control of the site. In such a situation, every decision made on the ground will be examined in retrospect: who made it, whether they had authority, and whether it can be shown that the site was managed in a controlled manner.
4. A foreman has ceased to serve in his position.

One of the key mechanisms in the regulations is the chain of response to unacceptable risk: identification, reporting, action, and approval.
When a risk is identified but not addressed in a timely manner, the failure is not localized but systemic. The mere identification of the risk is not sufficient, and responsibility is assessed based on the time it takes to respond, the quality of the treatment, and the manner of documentation.
In such cases, the question is not only whether a response was provided, but whether the response was provided in a timely manner, whether it was satisfactory, and whether it can be shown that an informed and documented decision was made.
5. Unacceptable risk not addressed in a timely manner

In each of these scenarios, the question is not only what happened, but whether it can be demonstrated that there was actual control over the risk, that a real-time decision was made, and that documented evidence of this exists.
The scenarios outlined above are not exceptional; they are typical situations arising from a gap between regulatory requirements and actual on-site practices.
In many cases, the failure does not stem from the absence of procedures, but from the lack of consistent implementation—whether due to schedule pressures, lack of coordination between parties, or reliance on retrospective completion.
What all these scenarios have in common is that they do not develop gradually, but occur at critical points where immediate action is required: appointment, response, approval, or documentation. When such action is not taken in time, the gap quickly escalates from a management exposure to an operational issue, and subsequently to legal and insurance exposure.
The implication is that the ability to prevent these failures does not depend on adding more procedures, but on building a working mechanism that ensures functional continuity, real-time response, and ongoing documentation of operations.
